OR Observers and Visiting Professors
For any visitor who wishes to shadow a physician/clinical staff
member and observe patient care at the University of Florida Health Science
Center and/or Shands HealthCare at UF, the following must be done PRIOR TO
THE VISIT:
1. A Request to Observe Patient Care form (see attachment) completed and
signed by the physician/ clinical staff member.
2. HIPAA training for visitors and vendors entering UF patient care areas
under the supervision of the UF Health Science Center. See Visitors and
Vendors: HIPAA at UF located beneath the Level I heading.

The visitor should provide 0000-xxxx (last 4 digits of Social Security
number) as the UFID#. Once the session is completed, please print out a copy
of the certificate.

HIPAA Confidentiality Statement
must be completed and physically signed by the visitor.
4. If the guest will have exposure to donated human anatomical specimens, a
Pledge of Respect for the Sanctity of Donated Human Anatomical Specimens
must be signed. This form is available from Mary Newman.
5. If your guest is going to be here more than 3 weeks, volunteer paperwork
must be completed. Foreign guests must have the appropriate visa status - J1
or H1. Contact Mary Ann for the appropriate forms and information.
6. The original Request to Observe Patient Care, the HIPAA Online Training
Certificate, and HIPAA Confidentiality Statement must be submitted to Dr
Good's office (PO Box 100192, Room M107 MSB) prior to the visit.
7. A copy of the Request to Observe Patient Care, a copy of the HIPAA Online
Training Certificate, a copy of the HIPAA Confidentiality Statement, and the
original Pledge of Respect for the Sanctity of Donated Human Anatomical
Specimens, if applicable, must be delivered to Mary Newman in the Chairman's
Office.
If volunteer paperwork had to be completed on your observer, please send a
copy of all the above to Mary Ann Hoyt in addition to Mary Newman.
Please note that observers under the age of 18 who are not enrolled at UF
are prohibited from observing/shadowing in patient care areas.
