Accident Reporting
If you are injured while on the job, please notify Joanie Nyland at 265-0077 immediately. She will contact the University’s Compensation office at 392-4940. After hours or weekends please call Corvel, the program administrator, at 1-800-929-01017 directly. Shands Hospital and the UF College of Medicine physicians are part of the Workers’ Compensation provider network. The following paperwork will be completed.
University of Florida - Authorization for Medical Attention workers’ Compensation Form WC-999 -To be completed only if the employee needs medical attention.
Notice of Injury - LES Form BCL-1 -This form must be completed when reporting any accident.
Occupational Injuries Investigation Report - This form must be completed by Joanie Nyland with the assistance of the injured employee, for all accidents.
