Medical Record Information Release
Before medical information from patient
medical records can be released to patients or third parties, a consent form
approved by the patient must be completed and sent to Health Information and
Record Management for processing. No other departments or individuals are
authorized to release medical record information to patients or third
parties. All requests should be written, and sent to Health Information and
Record Management along with a signed release form.
