Laboratory Orders
All written ancillary orders must be legible since they must be transcribed.
Radiology Orders - Requirements
• Patient name
• Patient medical record number
• Patient social security number or patient date of birth
• Test being ordered
• Written text for signs, symptoms, or diagnosis that relates directly to the reason the study is being ordered. The correlating ICD-9 code should be provided.
• Attending's name and number along side the ordering PA, ARNP, or • Resident's signature. (RNs cannot order without the Attending, PA, ARNP, or Residents signature.)
