Preferred method of applying:
We have a Microsoft Word version of our program
application. You can fill out the application, save it, and send it to us as
an e-mail attachment. Please also send as attachments any requested photos
or other documents.
We also have an HTML Version if you do not wish
to use the Word version. This should be filled out and then printed. You can
then mail it or fax it (along with the other items) using the above address
or fax number.
Our program is only able to certify anesthesiologists. However, individuals
from other specialties are invited to apply, but please note ---- Per ACGME
requirements, we are only able to accept J-1 visa holders who are trained in
anesthesiology, and who are therefore eligible to sit for the Board Exam
through the American Board of Anesthesiology.
